No worries - the clattering sound on startup is classic sign of a variator fault.
If your mechanic has another reason to think the head gasket has gone then all well and good - but if he is making this decision based on the clattering sound, then he is most likely wrong!
If in doubt - do you have access to an Alfa specialist who knows these engines? You don't want to be left with a damaged engine, or to be spending money unnecessarily... so perhaps an expert opinion is the best way to go.
